Connecting Care for Children (CC4C) is based within the children’s health team at St Mary’s Hospital, part of Imperial College Healthcare NHS Trust. Connecting Care for Children aims to bring together citizens, local organisations and child healthcare provider to enable the transfer of specialist child health knowledge from the hospital to the community. There is information and guidance and range of resources to improve pathways and Young People and family experience of them.
